Registration for Minors (Under 18 years old)

As of 11/08/2020, if you were born later than 11/08/2002, you are considered a minor (under 18 years old).

If you are a minor, in addition to the form you fill out for yourself, you:
-must submit a photo ID of your legal guardian/parents/legal representative.

*This applies for Singaporean citizens, foreigners residing in Singapore, and Residents outside of singapore.

Your legal guardian must fill out this form. They must state:
-Their relationship to you (Mother, Father, Sister, Brother, etc.)
-Name
-NRIC Number
-Valid Phone Number
-Valid Photo ID (only NRIC Card and Passports are accepted)

Your legal guardian MUST READ AND AGREE to our terms in order for you to register for donation payout.

*Please note, if there are any discrepancies between the info you input and the documents we receive, we may rescind your registration with information on why it was rescinded.
